Olive Leaf Extract Supplements: How to Choose One That Works
The difference between an olive leaf extract supplement that does something and one that does nothing is a single number, and most labels leave it off. Oleuropein content is what the trials measured, not milligrams of leaf. The clearest illustration comes from a 2025 meta-analysis in Phytotherapy Research, where pooling all doses gave a systolic blood pressure reduction of 6.03 mmHg with high heterogeneity, but restricting to trials using 1,000 mg a day gave 11.45 mmHg with zero heterogeneity (Lachovicz et al., 2025). Same ingredient. Different dose. Different answer.
TL;DR:
Look for: oleuropein content stated in milligrams per serving. The successful trials delivered roughly 120 to 160 mg of oleuropein a day.
Dose: 500 to 1,000 mg of standardized extract daily. The blood pressure effect was clearest at 1,000 mg.
Best evidence: lowering blood pressure and triglycerides in people who already have elevated readings.
Do not expect: blood sugar control, weight loss, or proven immune protection. Pooled human trials do not support those.
Main caveat: olive leaf extract adds to blood pressure medication and may add to diabetes medication.

What Olive Leaf Extract Supplements Contain
Olive leaf extract is made by concentrating the phenolic compounds from the leaves of Olea europaea. The compound carrying most of the research is oleuropein, with hydroxytyrosol second. Olive trees put far more oleuropein into their leaves than into the fruit or the oil, which is why the leaf is extracted separately rather than treated as a byproduct of harvesting olive trees for oil.
Commercial olive leaf extract supplements vary enormously, and in ways the front of the bottle does not reveal. Two products can both say "olive leaf extract 500 mg" while delivering 6 mg and 100 mg of oleuropein respectively, because standardization percentage, extraction method, harvest season, and leaf source all change the phenolic content. Some products are made from spent leaves left over after oil production, which are lower in phenolics than fresh leaf.
How to Choose: Six Things to Check
- Oleuropein in milligrams, not just a percentage. A label saying "standardized to 20% oleuropein" on a 500 mg extract gives you 100 mg. A label with no percentage at all gives you no way to compare it to any trial. This is the single most useful thing to check.
- Amounts you can actually reach. The trials with the strongest heart health results used 1,000 mg of extract a day. If a product provides 250 mg per capsule and suggests one capsule daily, the amounts fall to a quarter of the studied dose.
- Extract, not powdered leaf. Whole ground olive leaf is far weaker than a concentrated extract. Both are legitimate products; only one matches the research.
- Third-party testing. The Food and Drug Administration regulates supplements for safety and labelling rather than approving them for efficacy, so nobody verifies the oleuropein number before sale. Independent testing is the only practical check that the content matches the claim.
- A short, legible ingredient list. Proprietary blends hide how much olive leaf extract you are actually getting. If olive leaf sits inside a blend with six other botanicals and one total weight, you cannot dose it.
- Sensible combinations. Pairing olive leaf with another polyphenol such as quercetin is defensible, since they act through overlapping antioxidant pathways. Pairing it with a dozen unrelated ingredients usually means each one is underdosed.

What Olive Leaf Extract Supplements Actually Do
Blood pressure: the best-supported use
A systematic review and meta-analysis of 12 trials in 819 participants found olive leaf extract lowered systolic blood pressure by 3.86 mmHg overall, and by 4.81 mmHg systolic and 2.45 mmHg diastolic in people with hypertension (Razmpoosh et al., 2022, Diabetology and Metabolic Syndrome). The largest single trial randomized 621 people already treated for hypertension and found 24-hour systolic blood pressure fell 6.4 mmHg over 12 weeks, with reduced blood pressure load and improved lipid profile (Lamti et al., 2025, Journal of Hypertension).
A 2026 trial combining 1,000 mg of olive leaf extract providing at least 160 mg oleuropein with 300 mg of potassium, in 70 adults with untreated mildly to moderately elevated blood pressure, found morning systolic blood pressure on home monitoring fell 5.4 mmHg more than placebo over 12 weeks. Total cholesterol dropped 11.1 mg/dL, LDL 6.9 mg/dL, triglycerides roughly 22 mg/dL, and oxidized LDL about 23%, with fasting insulin and HOMA index also declining (Fladerer-Grollitsch et al., 2026, Phytomedicine).
Cholesterol and triglycerides: real but small
Reductions in triglycerides of around 9.5 mg/dl and total cholesterol of around 9 mg/dl in people with hypertension are the typical findings (Razmpoosh et al., 2022). A meta-analysis of oleuropein, hydroxytyrosol, and tyrosol across 14 studies found significant reductions in total cholesterol, triglycerides, and insulin, with subgroup analysis suggesting more benefit in people with a BMI under 30 and in those with existing cardiometabolic disease (Frumuzachi et al., 2025, Critical Reviews in Food Science and Nutrition).
Not every trial reproduces this. An 8-week study in 77 overweight adults with mildly elevated cholesterol found 500 mg a day changed nothing significant in blood lipids, oxidized LDL, blood pressure, glucose, insulin, or liver function (Stevens et al., 2020, European Journal of Nutrition).
Blood sugar and type 2 diabetes: not established
This is where honest testing of the evidence matters most, because olive leaf extract supplements are widely marketed for blood sugar management. The 12-trial meta-analysis found no meaningful change in glucose homeostasis (Razmpoosh et al., 2022). A 2026 meta-analysis of 11 randomized trials concluded that current evidence does not support clinical recommendations for olive leaf extract supplementation, and that in continuous-intake parallel-design trials there were no significant effects on glycemic or lipid outcomes (Menezes et al., 2026, Food and Function).
People with type 2 diabetes should read that in both directions. Olive leaf extract is not a type 2 diabetes treatment, and the reductions in insulin seen in some trials are still real enough to matter alongside diabetes medications. Diet remains the larger lever for blood sugar management than any capsule.
Immune support: laboratory only
Oleuropein and its metabolite elenolic acid inhibit bacteria, fungi, and viruses in cell culture testing. That is where the immune claims come from. No randomized trial has shown olive leaf extract prevents or shortens any infection in people, and no olive leaf product is approved to prevent or treat disease.
Dosage and How to Take It
Aim for 500 to 1,000 mg of standardized extract a day, with 1,000 mg the amount that produced the clearest blood pressure result. In oleuropein terms, the successful trials delivered roughly 120 to 160 mg a day. Split it morning and evening if the product allows, and take it with food to reduce stomach upset. Olive leaf is not a stimulant and the trials reported no effect on sleep, so evening dosing is fine.
Give it six to twelve weeks. Every trial that measured blood pressure ran at least six weeks, and the largest effects appeared at week 12. If you are tracking blood pressure at home, take readings the same way each morning, because the 2026 combination trial found home monitoring detected a difference that office readings did not.
Who Should Not Take Olive Leaf Extract
- Anyone on blood pressure medication without their prescriber's knowledge. This is the interaction with real consequences: adding a hypotensive supplement to hypotensive medication can cause dizziness, fainting, and falls.
- Anyone on insulin or oral type 2 diabetes medications. Some trials reduced fasting insulin and glucose. Monitor and involve your prescriber.
- Anyone on warfarin or antiplatelet drugs. Polyphenols can affect platelet function.
- Pregnant or breastfeeding women. There is not enough safety data.
- People allergic to olive pollen. Olive pollen is a common allergen, and anyone who reacts to it should start cautiously or avoid olive leaf entirely.
- Anyone within two weeks of surgery. Stop because of the blood pressure and platelet effects.
- People with low blood pressure already. There is no benefit to lowering it further.
Side effects at studied doses are mild and mostly digestive. The 621-person hypertension trial reported no significant adverse events over 12 weeks (Lamti et al., 2025).
