Choline Supplement for Pregnancy: Dose, Evidence and Forms
Choline is the nutrient most prenatal vitamins skip, and most pregnant women fall short of it. A 2022 systematic review and meta-analysis in Advances in Nutrition pooled case-control data from 1,131 mothers of newborns with neural tube defects and 4,439 control mothers, and found low maternal choline intake was associated with higher odds of a neural tube defect, with a pooled estimate of 1.36 (Obeid et al., 2022). The same review concluded higher maternal intakes of 550 mg to 1 g a day were safe and likely to help several domains of child neurocognition. The cognitive evidence is genuinely mixed, though, and this article covers both sides.
TL;DR:
Recommended intake: 450 mg a day in pregnancy and 550 mg while breastfeeding. Average intake is roughly 70% of that.
What trials tested: 480 mg versus 930 mg a day in the third trimester. The higher dose produced faster infant information processing and better sustained attention at age 7.

Form: choline bitartrate is what the trials used. Most prenatal vitamins contain little or no choline, so check the label.
Main caveat: above 3,500 mg a day choline causes fishy body odor, low blood pressure, and sweating. Talk to your obstetric provider before supplementing.

What Choline Is and Why Pregnancy Changes the Math
Choline is an essential nutrient your body makes only in small amounts, so most has to come from the diet. It has three jobs that matter in pregnancy. It builds phosphatidylcholine, a structural part of every cell membrane. It makes acetylcholine, the neurotransmitter behind memory and attention. And it donates methyl groups, feeding the same one-carbon metabolism pathway as folate and vitamin B12. That third role is why choline sits close to folate in the neural tube story: both supply methyl groups while the neural tube closes in the first weeks, often before a woman knows she is pregnant.
Demand then rises sharply. The fetus draws choline across the placenta against a concentration gradient, and amniotic fluid choline runs about ten times higher than maternal blood, depleting the mother's own stores. Lactation raises the requirement again, because breast milk is rich in choline.
What Is the Recommended Choline Intake in Pregnancy?

These are Adequate Intake values rather than Recommended Dietary Allowances, which matters: an AI is set when evidence is too thin to calculate an RDA. It is a best estimate, not a precise target.
The gap between recommendation and reality is the practical problem. Researchers running the seven-year follow-up noted that choline consumption by pregnant women averages around 70% of the Adequate Intake (Bahnfleth et al., 2021, The FASEB Journal). Most prenatal vitamins contain little or no choline, because it is bulky and would make the pill too large to swallow.
The Evidence on Choline and Brain Development
The rodent work is unambiguous: prenatal choline deprivation causes lifelong cognitive deficits in offspring and supplementation produces lifelong benefits. Human research is where it gets complicated.
The controlled feeding trial. Women entering their third trimester were randomized to 480 mg or 930 mg of choline a day until delivery, with all food supplied so intake was tightly controlled. Infants were tested at 4, 7, 10, and 13 months, and mean reaction time across those ages was significantly faster in the 930 mg group. Within the 480 mg group, longer exposure predicted faster reaction times, suggesting even modest increases help (Caudill et al., 2017, The FASEB Journal).
The seven-year follow-up. Twenty of those children were retested at age 7 on a sustained attention task. The 930 mg group scored better on the primary endpoint and maintained correct signal detections better across a 12-minute session. On the briefest signals, the 480 mg group's hit rate declined 22.9% across the session while the 930 mg group's rose 1.5% (Bahnfleth et al., 2021). A 14-year adolescent follow-up of the same cohort is underway (Roth et al., 2025, JMIR Research Protocols).
The trial that found nothing. In a double-blind randomized trial, 140 pregnant women took 750 mg of phosphatidylcholine or a corn oil placebo from 18 weeks gestation through 90 days postpartum. Their infants showed no significant differences in global development, language, or memory at 10 and 12 months. Those women were already eating about 360 mg of choline daily from food, roughly 80% of the recommended intake (Cheatham et al., 2012, The American Journal of Clinical Nutrition).
The systematic review. A 2025 review in Nutrients examined four randomized trials and five observational studies across 20 publications. Some trials found benefits on individual measures, but each reported multiple outcomes and most were null. The authors concluded current evidence is insufficient to support or refute the idea that more choline in pregnancy improves child neurodevelopment (Gould et al., 2025, Nutrients).
Reading these together: meeting the Adequate Intake is well justified, because shortfall is common and the neural tube association is real. Pushing to double the AI for a cognitive edge rests on one small feeding trial with promising follow-up, not on settled science.
Choline and prenatal alcohol exposure
One population has clearer evidence. In 69 heavy-drinking women in Cape Town randomized to 2 g of choline daily or placebo from mid-pregnancy, infants in the choline arm were more likely to meet criterion on eyeblink conditioning, showed catch-up growth in weight and head circumference, and had better visual recognition memory at 12 months (Jacobson et al., 2018, Alcoholism: Clinical and Experimental Research). A meta-analysis of five trials found prenatal choline likely benefits memory in alcohol-exposed pregnancies, standardized mean difference 0.61 (Lino et al., 2025, Pediatrics and Neonatology).
Choline-Rich Foods Come First
- Two large eggs: roughly 300 mg, almost all of it in the yolks
- Beef liver, 3 oz: roughly 350 mg
- Chicken breast, 3 oz: roughly 72 mg
- Salmon, 3 oz: roughly 60 mg
- Soybeans, half cup: roughly 55 mg
- Broccoli or Brussels sprouts, half cup: roughly 30 mg
Two eggs plus a normal mixed diet lands most people near 450 mg. Women avoiding eggs, eating vegan, or dealing with early-pregnancy food aversions are the ones most likely to fall short. Liver is very high in preformed vitamin A, so limit it during pregnancy despite the choline.
Choline Supplement Forms and How to Take Them
Not all choline supplements are equivalent, and the pregnancy evidence sits with one form.
Choline bitartrate is the workhorse: inexpensive, about 41% choline by weight, and the form the alcohol-exposure trial used. If a supplement lists 500 mg of choline bitartrate, that delivers roughly 205 mg of actual choline. Check which number the label quotes.
Phosphatidylcholine is the main dietary form and the one used in the 2012 trial that found no cognitive benefit. It is gentler on the stomach and lower in elemental choline per gram.
Citicoline (CDP-choline) and Alpha-GPC are the nootropic forms, chosen for how efficiently they cross into the brain. Safety, Side Effects, and Too Much Choline
The tolerable upper limit is 3,500 mg a day for adults, including in pregnancy. Above it, the documented effects are fishy body odor from trimethylamine, sweating, excessive salivation, low blood pressure, and liver toxicity at extreme intakes. Below it, complaints are mild nausea and stomach upset, which food usually solves.
Some points worth raising with your provider:
- Choline deficiency symptoms are non-specific: fatigue, low mood, memory complaints, and muscle aches. There is no routine blood test for choline status, and plasma choline is a poor marker of intake, so deficiency is inferred from diet rather than measured.
- Fishy odor at ordinary doses can indicate trimethylaminuria, a genetic condition affecting how choline is metabolized. If it happens, stop and mention it to your clinician.
- TMAO and heart health. Gut bacteria convert some choline to TMAO, associated with cardiovascular risk in observational research. The relevance to pregnancy is unclear, and it is a reason not to take far more than you need.
- MTHFR variants. Since choline and folate share the methylation pathway, choline demand may rise when folate metabolism is impaired. That is a conversation for your provider, not a reason to self-prescribe a high dose.
