Astragalus Benefits: What the Human Studies Support
Astragalus is one of the most-used herbs in traditional Chinese medicine and one of the most over-extrapolated in Western supplement marketing. The gap sits between laboratory work, which is extensive, and human trials, which are fewer. A 2023 systematic review and meta-analysis of 19 human studies in 1,094 participants found astragalus significantly increased CD3 levels and the CD4/CD8 ratio, two markers of cellular immune function, while reducing several pro-inflammatory cytokines, with the authors noting substantial heterogeneity between studies (Zhang et al., 2023, Complementary Medicine Research).

What Astragalus Is
Astragalus root comes from Astragalus membranaceus , a legume-family plant native to northern China and Mongolia. In traditional Chinese medicine it is called Huangqi and classified as a qi tonic, used for fatigue, poor appetite, and frequent illness, usually as a decoction of the sliced root or as astragalus root tea. It is one of the herbs that crosses the food and medicine line in Chinese practice, appearing in soups as often as in prescriptions.
The active constituents fall into three groups. Astragalus polysaccharides (APS) carry most of the immune research. Saponins, chiefly astragaloside IV, and flavonoids make up the rest. Commercial astragalus extract is usually standardized to polysaccharide content, sometimes to astragaloside IV, and occasionally to nothing at all.
Note that astragalus root is the medicinal part. Astragalus leaves are not the traditional preparation and are not what the research studied.
Astragalus Benefits: Where the Human Evidence Is
Immune system markers
This is the strongest area. The 19-study meta-analysis found astragalus significantly raised CD3 levels and the CD4/CD8 ratio, standardized mean difference 2.46, while lowering IL-2, IL-4, IL-6, IL-10, TNF-alpha, and interferon gamma (Zhang et al., 2023). Raising cellular immune markers while lowering inflammatory cytokines is the pattern people mean when they call astragalus an immune modulator rather than an immune booster.
A larger and more specific body of work exists in cancer care. A systematic review and meta-analysis of 31 randomized controlled trials in 2,648 patients with malignant tumors found astragalus polysaccharide supplementation improved objective response rate, with a risk ratio of 1.30, and raised CD3+ T cell counts, CD4+ T cell counts, and the CD4+/CD8+ ratio, with a low incidence of adverse reactions (Li et al., 2025, Naunyn-Schmiedeberg's Archives of Pharmacology). That is a meaningful body of clinical evidence, and it belongs to supervised oncology settings rather than to self-prescribed supplement use.
Exercise, fatigue, and post-exercise immune suppression
Hard training temporarily suppresses immune function, and astragalus has been tested against that. In a double-blind study, 18 members of the Polish Rowing Team took 500 mg of astragalus membranaceus root or placebo through a six-week intensive training camp. The supplemented rowers maintained natural killer cell and regulatory T cell levels at baseline while the placebo group's declined, and showed a modulated IL-2 response during recovery (Latour et al., 2021, Journal of the International Society of Sports Nutrition).
A systematic review of 11 clinical studies in sport found astragalus supplementation, typically 1 to 4 g a day of dried extract over several weeks, was associated with improved aerobic performance, better oxidative status, and better post-exercise immune function, particularly natural killer cell activity and the CD4+/CD8+ ratio, without improving strength and without reported adverse effects (Antonelli et al., 2024, Encyclopedia).
Inflammation and airways
An extract standardized to 16% polysaccharides reduced prostaglandin E2, leukotriene C4, IL-1 beta, TNF-alpha, and nitrite production in macrophages, and improved lung function measures in an animal model of allergic airway inflammation (D'Avino et al., 2023, International Journal of Molecular Sciences). That is laboratory and animal work. It explains why astragalus is marketed for lungs and allergies; it does not show that it helps a person with asthma.
What the reviews say about everything else
Large reviews catalogue immune organ effects, anti-tumor activity, antiviral effects, blood glucose regulation, lipid-lowering, anti-fibrosis, and anti-radiation properties for astragalus polysaccharide (Zheng et al., 2020, Frontiers in Pharmacology ; Li et al., 2022, Archives of Pharmacal Research). Read those lists carefully and you will find almost all of it is cell culture and animal research. The 2022 review states the position plainly: astragalus polysaccharide is a valuable immunomodulatory medicine, but further high-quality studies are warranted to corroborate its clinical efficacy.
Specifically, astragalus has not been shown in randomized trials to prevent colds or flu in healthy adults. A trial designed to test that question in people with recurrent upper respiratory infections was registered in 2024 and its results are not yet published (Cheon et al., 2025, BMC Complementary Medicine and Therapies).
Dosage and How to Take Astragalus
Human studies used a wide range. The sports review found 1 to 4 g a day of dried extract taken over several weeks; the rowing trial used 500 mg a day; traditional decoctions use 9 to 30 g of dried root simmered in water. A common supplement dose sits at 500 to 2,000 mg a day of root extract.
Standardization matters more than the total milligrams. Look for a stated polysaccharide percentage or astragaloside IV content, since a product with neither gives you no way to compare it with the research. Take it with or without food, and expect to give it four to six weeks, since the trials that measured immune markers ran six weeks or longer.
Astragalus root tea is the traditional route and is genuinely pleasant, faintly sweet, and easy to add to soups and stocks. It delivers less than an extract and is a reasonable everyday habit. Side Effects and What Not to Mix It With
Astragalus is well tolerated in the human studies, with the cancer meta-analysis reporting a low incidence of adverse reactions and the sports review reporting none (Li et al., 2025; Antonelli et al., 2024). Mild digestive upset and, rarely, a rash are the usual complaints. The interactions matter more than the side effects.
- Immunosuppressant drugs. This is the important one. Astragalus stimulates immune activity, which works directly against cyclosporine, tacrolimus, and other transplant medicine. Do not combine them.
- Autoimmune conditions. Lupus, rheumatoid arthritis, multiple sclerosis, and Hashimoto's involve an already overactive immune response. Stimulating it further is a conversation for your specialist, not a self-directed experiment.
- Cancer treatment. Astragalus polysaccharide has real trial evidence as an adjunct in oncology, and that is precisely why it should be your oncologist's decision. Some herbs interact with specific chemotherapy agents.
- Blood thinners. Astragalus may have mild antiplatelet activity, so combining it with warfarin or antiplatelet drugs warrants medical input.
- Diabetes medicine. Blood glucose effects appear in the animal literature. Monitor if you take insulin or oral agents.
- Blood pressure medicine. Astragalus may lower blood pressure modestly, which can add to antihypertensives.
- Pregnancy and breastfeeding. There is not enough safety data. Skip it.
One further caution specific to this plant: several Astragalus species other than membranaceus are toxic, including locoweed varieties that accumulate selenium. Buy from a supplier that identifies the species on the label.
